On Healthcare Beans, James Haven speaks with Nate White, founder and CEO of Cibolo Health, about a different path for rural healthcare: gaining scale, infrastructure, and value-based care capability without giving up local ownership.

Before founding Cibolo Health, Nate served in senior leadership at Sanford Health during a period of major system growth. He was also named to the 2026 TIME100 Health list.

Cibolo supports networks of independent rural hospitals and clinics that share the infrastructure needed for clinically integrated networks and value-based care — including data, analytics, contracting, compliance, clinical support, and other capabilities.

The networks themselves remain owned and governed by their members, with a one-member, one-vote structure that gives smaller rural providers the same formal voice as larger participants.

The conversation explores why fee-for-service economics are especially difficult in rural markets, where patient volume, population growth, and commercial insurance mix are often limited.

It then turns to one of the more surprising arguments in the episode: rural providers may, in some cases, be particularly well aligned with total-cost-of-care incentives.

For many rural hospitals, a substantial share of attributed healthcare spending occurs outside their own walls. Nate argues that value-based care creates an opportunity to think differently about that flow of care — strengthening services that can appropriately remain local while bringing cost and quality more explicitly into referral decisions when patients need higher-level care elsewhere.

That is also why Nate pushes back on the idea that Cibolo’s primary value is negotiating leverage. The goal is not simply to aggregate rural hospitals so they can demand higher prices. It is to give independent providers enough shared infrastructure and scale to participate meaningfully in value-based care while preserving local control.

James and Nate also discuss:

  • Scale without consolidation
  • Member ownership and equal-vote governance
  • Rural patient outmigration and referral economics
  • Why negotiating leverage is not the core Cibolo strategy
  • Cost and quality information in referral decisions
  • Rural Health Transformation funding
  • The limits of price transparency without consumer financial incentives
  • AI as a decision-support layer for value-based care
  • What Cibolo hopes rural healthcare looks like five years from now
